PHapp® sends free, local emergency and public-health alerts. We're evaluating how useful it is for our community, and we need your honest experience to make it better.
What's involved
- Download PHapp® — It's free, on iPhone or Android, and takes a minute to set up. Use it for about a month.
- Take a 5-minute survey — At the end of the month, tell us what was useful and what wasn't. The first 200 participants earn a $15 gift card.
- Optional: join a focus group — A friendly, in-person conversation at Mariposa Health Center in Nogales (or in Patagonia). The first 40 participants earn a $30 gift card.
- Completely voluntary — Take part on your own schedule. This is a program evaluation, not research, and you can stop at any time.
Who can take part
We're recruiting two groups across Santa Cruz County.
- Community Health Workers — Promotoras and frontline staff at Mariposa Health Center, CHAC, County HHS, and partner organizations.
- Community members — Anyone who lives or works in Santa Cruz County, is 18 or older, and has a smartphone.

How much can I earn?
The first 200 people to finish the 5-minute survey earn a $15 gift card. The first 40 people to join a focus group earn a $30 gift card. You're welcome to do both.

What will I need to do?
Download PHapp® (it's free), use it for about a month, and take a short survey at the end. If you'd like, you can also join a small in-person focus group.

Who can join?

Is this a research study?
No. This is a program evaluation to make PHapp® and local alerts more useful for Santa Cruz County. Taking part is completely voluntary, and you can stop at any time.

Who's running this evaluation?
It's run by Hub — the team behind PHapp® — together with the University of Arizona and the Arizona Department of Health Services, alongside Santa Cruz County health partners.

Is my information private?
Yes. What you share is used only for this evaluation. We don't sell your information or show ads, and your feedback isn't tied to your identity in any report.

Where are the focus groups?
In person at Mariposa Health Center in Nogales, with sessions also planned in Patagonia. Each is a relaxed, roughly 45-minute conversation — we'll work around your schedule.

When do I get the gift card?
After you complete the survey, or after you take part in a focus group. We'll send it to the email or phone number you give us.
